The Young Adults Team work with young people with disabilities aged 16-25. The work is primarily with young people with Learning Disabilities, but also some young people with a physical disability or complex health needs,  to transition from Children’s Services into their adult life.  The work is very varied, and requires working in a multi-agency manner with Worcestershire Children First(WCF), Special Educational  Needs Department (SEND), health colleagues, schools and colleges to ensure young people get the whole range of options they deserve when moving into adulthood.  The work involves using Liquidlogic in both the adult and children’s systems, and using both children and adults legislation effectively.  We cover all of Worcestershire, with some placements nationally so any applicant would need the ability to travel.

We have a dedicated Housing Officer to support workers when looking at an accommodation offer for young people, and an innovative Carer Support worker to link in with their families and support networks.  The team have a range of specialisms and experience to support any new workers, in addition to weekly reflective practice sessions and positive management support.

The role is for a qualified social worker at 28 hours weekly.
