An excellent opportunity has a risen within the Carers Network Team (part of Dudley Carers Hub and Wellbeing Service) for a permanent Assistant Care Coordinator

The team supports individuals who provide care and support to partner, family member or friend. We are committed to raising awareness of carers living in Dudley and ensuring they receive the right information, advice and support.

Hours: 37 per week

Salary: Grade 6

We looking to recruit a self motivated individual to join the Carers Network team 

Based in a dedicated team your primary role will be to assess the needs and contribute to the development of support plans of unpaid carers who provide care and support to a vulnerable person. You will be required to carry out assessments in the community, focusing on timely interventions to maximise the independence and wellbeing of individuals accessing the service to achieve positive outcomes.

The role is varied and no two days are ever the same. We operate operate a duty system that you will be part of to provide a streamlined customer journey. Part of your daily role will involve speaking to individuals, face to face, telephone and online providing information, advice, completing carers and social care assessments, developing support plans to achieve positive outcomes.

We are looking for a candidate who

  • has proven experience of assessing individuals needs  with the ability to capture and interpret information and deal with challenging and complex situations in a sensitive and creative manner
  • is able to establish, maintain and develop effective working relationships with colleagues, carers, advocates and key stakeholders
  • has experience of using a wide range of IT packages/systems and internet reference sites
  • is self-motivated. organised with effective communication skills both written and verbal and able to working under pressure in a fast paced environment

This post is agile and you will be required to work at an number of locations across the borough and in the community.

Applicants need to hold a full driving licence to travel around the borough, and maybe required to carry demonstration equipment in their vehicle.

The post is subject to medical clearance and an enhanced DBS check.
