An exciting opportunity has arisen for an Advanced Social Work Practitioner to work within the Onward Care Team based at Alexandra Hospital in Redditch. This is a full-time position, 37 hours per week.

The Onward Care Team is based in Worcester Royal Hospital and the Alexandra Hospital. The team is a multi-disciplinary team consisting of Worcestershire County Council and Health and Care Trust staff. There is a focus on multi-agency partnership working with wider partners. The teams work closely with Worcestershire Acute Trust and continuing health care to support patient’s onward care.

The team works with older adults and adults 18 years and over who have diagnosis of dementia, people with physical disabilities, people with mental health diagnosis and mild to moderate learning disabilities/and/or autism.

The team works within the Care Act 2014 and the Mental Capacity Act 2005. The team ensures a focus on Adult safeguarding, ensuring organisational statutory duties are met. The team works with the 3 Conversation model focusing on strength-based practice.  The Home First Agenda and the 3 Conversation Model are key to our work, so it is important that you are creative and innovative in your approach to ensure that we achieve the aims and aspirations of those we work with.  Your role will be supporting this practice through formal and informal supervision.

The discharge to assess model is a key element to the hospital discharge process. As ASWP you will need to support the team with operating a discharge to assess model, supporting, and advocating positive risk taking. You will work closely with your team and partner organisations to gather information for complex care needs which require creative and joint up approaches to support discharge.

ASWP’s will have line management responsibility for a group of Social Workers/Social Care Workers and will be providing regular Supervision. There is an expectation that when required you will Chair Best Interest Meetings, so an understanding of the Mental Capacity Act and the role of the Court of Protection and Public Guardian is required.

The successful candidate will be expected to demonstrate strong Leadership Skills and are committed to supporting the Social Workers career progression. It would therefore be desirable that you have completed PE1 and PE2 or working towards this.
