7 x Social Workers for Telford Integrated Communication Assessment Team (TICAT)
Full-time - 37 hours per week (some weekend working)
Qualified / Experienced - Grade PO2 /PO3 (£32,234 to £35,745)
An exciting opportunity to work for a LA that’s recently won Exceptional Adult Social Care 2021 and Excellent Co-operative council of the Year 2021.
Following a pilot period we now have permanent positions for a part-time social worker and an adult practitioner (see separate advert).
This involves often crisis working with rapid response team, to prevent hospital admissions, supporting carers and families.
We use strengths and asset based approach in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support is required we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control.
We encourage positive risk taking with a ‘home is best’ philosophy, this is reflected in our performance as Telford and Wrekin is proud to have a lower than national average per population of admissions into long term placements for older people.
The Social Worker roles are expected to undertake statutory Social Work functions in line with the Care Act and the Mental Capacity Act. To undertake duties expected of Social Workers in line with the Professional Capabilities Framework and in accordance with the specific job description attached. The Social Worker role will work with complex Adults, their carers/family as directed by the Team Leader
