We have an exciting opportunity for an enthusiastic individual who really wants to make a difference with people with learning disabilities and Autism and their families.
If you are a great coordinator with fantastic communication skills and want to support the National Development Team inclusion (NDTi) small supports programme in Stoke-On -Trent than this may be the job for you.
The role will include working with individuals who will have a learning disability or Autism who want to move to their own home normally from a hospital setting working with families, commissioners, social workers, housing, community organisations, attracting new small providers.
