Worcestershire County Council Provider Service – Relief Pool

Relief Service Assessor

Rate of pay: £14.83 per hour

Our approach is to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We will continue to focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible.

This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life and remain independent for as long as possible.

We are looking to recruit to 10 posts of Relief Service Assessor.

As a Relief Service Assessor, you will be engaged on an as and when basis to help us meet demand when there are additional pressures.

You must be able to:

  • Undertake initial assessments within the community; identifying needs, risks and the service user's potential for regaining independence with personal and / or domestic activities of daily living
  • Develop Care Plans that capture the desired service user outcomes and detail the required intervention of the service    
  • Actively case manage service users, as part of a multidisciplinary team, within a locality including regular reviews of their progress (if required as part of the engagement)
  • Ensure that all standards and performance measures are adhered to, including service user monitoring protocols and quality assurance audit completion.

As the ideal candidate you should have a level 3 Diploma in Health and Social Care or considerable experience of working in a social care setting.  It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate and empathetic.

As part of the Provider Services Relief Pool you may be asked to support services at short notice. While you are not obliged to do this, we are looking for people who are willing to be flexible and responsive to requests.
