ConnectWELL Link Worker
NHS Band 5 point 18 £24,214 pro rata
21 hours per week
Initial contract until 31st March 2022 with possible extension
An exciting opportunity has arisen to join ConnectWELL’s award winning Social Prescribing programme. The Rugby Health Primary Care Network and Warwickshire Community and Voluntary Action are working together to extend social prescribing provision for patients in the local area. They are looking for a proactive and committed individual to join the small and friendly ConnectWELL Team.
ConnectWELL is a free service to help people improve their general health and wellbeing their way. Through using non-medical sources of community support, ConnectWELL offers the chance to find out about activities that can improve health and wellbeing, including: advice and information services, community groups, leisure activities, lunch clubs, self-help groups, specialist interest groups, sporting activities, and lots more.
Main duties will include developing relationships in the health and social care sector, and supporting individuals to access voluntary and community opportunities.
Excellent communication and organisational skills, the ability to work on own initiative and as part of a team are essential for this role.
