1 permanent full-time role - 37 hours per week
1 fixed term (2 years) full-time role - 37 hours per week
Scale 5 - £22,627 to £24,491

We have two locality teams based in our communities working with older people, physically disabled and those with an acquired brain injury.

We use strengths and asset based approach in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support is required we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control.

We encourage positive risk taking with a ‘home is best’ philosophy, this is reflected in our performance as Telford and Wrekin is proud to have a lower than national average per population of admissions into long term placements for older people.

Our Adult Practitioners will work with the individual, their carers and families, to build a Support Plan, with a broad range of interventions that will provide a provision that is holistic, evidenced based and delivered within communities.
