Title: Home First – Shift Leader
Location: Hereford
Salary range – £21,166 – £24,799
Hours: 37.00

The shift leader is at the heart of the service. This is a fast paced and dynamic role, with many different functions that are key to the delivery of Home First. This role is all about organisation, prioritisation and planning … if you have these skills, this role is for you!
A Shift Leader with Home First plays a pivotal role in;
• Managing the internal scheduling system, managing service capacity based on the number of staff available, allocation of work to Reablement workers and assessing staff.
• Managing staff rotas, ensuring that the service is well staffed and can consistently meet the need of clients.
• Managing staff annual leave and reporting sickness
• Be the first point of contact for front line staff throughout the shift, providing advice and guidance.
• Ensuring the safety of the staff team and providing support where required
• Managing all referrals that come through the service - making decisions around correct pathways for clients to ensure support needs are met
• Liaising with Hospital teams, external agencies in relation to client support needs
• Allocating assessments and reviews ensuring these are completed in a timely manner
• Supporting the service to maintain a consistent “flow” of clients by ensuring that clients are being regularly reviewed and being discharged in a timely manner.
• Answering and responding to phone calls from internal and external services
• Supporting the Registered Manager with data collection around Service Key Performance Indicator

What we are looking for in the ideal candidate:

• Exceptional, planning and organisation skills
• The ability to manage multiple tasks at once, demonstrating strong prioritisation skills
• Excellent IT skills and experience of working with spreadsheets.
• A decisive decision maker who is assertive but fair in their approach to problem solving
• The ability to work confidently and calmly under pressure
• Self - Motivated to continually improve the quality and standard of work individually and that of the team.
• Fantastic intrapersonal skills, able to build good working relationships with others and can contribute positively working in a team
Essential experience and qualifications
• You must have experience of working in Health and Social care, ideally in a similar position.
• You must hold a level 2 or above in Health and Social Care
• You have experience of completing initial service assessment, risk assessments, support planning and reviewing the support needs of people that use care services.
• An understanding of the geographical area of Herefordshire

The shift leader is expected to work a shift pattern, which covers every other weekend.
