Herefordshire Council Adults & Communities are currently creating a new team, the focus of which is carrying out short term assessment & support planning with service users throughout Herefordshire who have recently left hospital and require ongoing services. This is an exciting opportunity to join our new team and we are looking for full-time, enthusiastic staff who enjoy fast paced work to set timescales and are keen to support service users and make a success of the new team.

We are looking for an experienced Manager to lead the new team. The successful applicant will have a professional qualification in Social Work and be registered with Social Work England and will have demonstrable management experience working in Health and Social Care community environments.

Working across Herefordshire will support our ambition to help adults live safe, healthy and independent lives by offering enhanced preventative and proactive community services. This means our multi-skilled team will enable us to be responsive to individuals who have recently left hospital and require ongoing services.

The work will be short term with set timescales therefore we are looking for an individual with excellent organisational skills who is able to work effectively at the interface between Locality Teams in Adult Social Care and Integrated Care Services.
