Our approach will be to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible.
This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life and remain independent for as long as possible.
To help us achieve our Team’s goals we would like an Experienced well-motivated Social Worker to join us. The successful candidate will need to be able to balance competing priorities and understand the 3Conversation model of Social Care delivery. You will be expected to work from a Strength Based perspective, using creative and innovative problem solving to achieve good outcomes for the residents of Worcestershire.  
It is essential that you have an extensive understanding of the Mental Capacity Act with case experience including an understanding of the role of Court of Protection. You should also have a working understanding of Continuing Health Care and the Care Act and the implications for providing services.
 As an Experienced Social Worker, you should be able to demonstrate an understanding of working with those Individuals who require Safeguarding and the complexities around this area of work. As part of your role you will also be expected to provide peer support to those less experienced. 
The successful candidate will receive regular supervision and Continual Professional Development opportunities from the Community Hospital Social Work Team as well as case management support from the Reablement Team.
