Occupational Therapist-   Adult Social Care First Contact Team

Salary: Grade 9 £32234 - £36922 per annum (pro rata)

 37 Hours per week – Fixed Term until March 2023 (Monday – Thursday 09:00 – 5:00 pm Fridays 09:00 am – 4:30 PM.

First Contact are an adult social care team made up of social care assessors and social workers who engage people in Care Act compliant assessments via the telephone or virtual technology which explore their social care needs and what assets are available to meet them.

First Contact aim to ensure that

  • People’s needs are understood at the earliest opportunity
  • Care and support services are arranged at the earliest opportunity
  • People and their families/carers have contact with the minimum number of social care professionals

The First Contact Team, engages with citizens of Staffordshire, carrying out strengths based, outcome focused assessments via the telephone. The Care Act 2014 compliant assessments determine citizens assessed eligible needs while promoting their assets and facilitating their independence.

The team is part of the wider Adult Social Care Team within the council

The Adult Social Care First Contact Team is seeking to employ an Occupational Therapist to join a team of Social Care Practitioners and Social Workers.

This key role will take responsibility for working across Older People, Physical and Learning Disabilities and as such we are seeking an experienced Occupational Therapist with a range of knowledge and skills across this broad agenda.  If you have a desire to support the Local authority in developing our approach to supporting citizens at the first point of contact, this role could be for you.

You will be responsible for engaging with citizens of Staffordshire, carrying out strengths based, outcome focused Occupational Therapy assessments remotely. You will determine citizens assessed eligible needs while promoting their assets and facilitating their independence by providing sound information and advice and making a judgement with regards to eligibility.

The role also includes signposting and referring onto other agencies such as Community-based OTs and working closely with our OT Colleagues in Midlands Partnership Foundation Trust regarding clinical decision making, referrals on Disabled Facilities Grant, referrals and ordering of low-level equipment as required to meet immediate needs.

This is also a unique and exciting opportunity which will also provide you with an opportunity to maintain your clinical assessment skills by providing cover to the Staffordshire County Council Prison Occupational Therapy service.

Staffordshire county council support the professional supervision of its staff by offering monthly management supervision and support to arrange appropriate professional/clinical supervision

The team is part of the wider Adult Social Care Team within the council

The Service Objectives are:

1. Deliver a practitioner-led, robust Adult Social Care First Contact for Staffordshire.

2. Promote effective information, Advice & Guidance signposting leading to earlier self-help and citizens maintaining independence for longer.

3. Increase knowledge of local provision and support at the front end of the Adult Social Care Pathway and connect citizens with community solutions at the earliest possible stage.

4. Inform citizens at the earliest opportunity of financial expectations related to the Adult Social Care Pathway.

5. Establish the correct resource and skill requirements to embed a robust, asset based, preventative approach at the front end of the Adult Social Care Pathway
