POST: Social Worker – Level 1 including NQSW entry level ASYE (band 5) – permanent

BASE: Civic Offices Cannock Chase, Beecroft Road Cannock WS11 1BG

HOURS: Fulltime 37.5 hours

TEAM: Cannock Community Team 2

An exciting opportunity have arisen within the Cannock Adult Social Care Community Team for a Level 1 registered Social Worker.

You will be part of the Cannock planned care community team working in partnership with colleagues within our wider care group and a good knowledge of the services available to support and promote independence in the home environment is essential.

The post holder will need to be a registered Social Worker and will have responsibility for completing statutory assessments under the Care Act 2014. The post will involve undertaking assessments, support planning, review and safeguarding enquiries of vulnerable adults. The successful applicant will be required to complete mental capacity act assessments and best interest decisions in line with the Mental Capacity Act, 2005. The remit of the service is to support the citizens of Staffordshire predominantly over the age of 18 with physical disabilities and older people over the age of 65 with physical and mental health disabilities.

We are looking for an enthusiastic, proactive and innovative individual who posses’ excellent communication and organisational skills and has a good working knowledge of the Care Act 2014 and Mental Capacity Act 2005. The successful candidate will have a flexible approach and be a team player who is able to forge excellent working relationships with multi-disciplinary teams and partner agencies.

You will be expected to work 08:30 – 17:00 (full time post) and have a full driving licence and access to a car.
