Adult Practitioner - Telford Integrated Community Assessment Team (TICAT)
Grade 5 - £23023 to £24920 per annum
Full-time - 37 hours per week - some weekend working

An exciting opportunity to work for a LA that’s recently won Exceptional Adult Social  Care 2021 and Excellent Co-operative council of the Year 2021

We use strengths and asset based approach in our practice; how individuals can be supported by the strengths they have in themselves, their network of support from family, friends and their local communities. Where care and support is required we work to ensure that this reflects the individual’s assessed needs and strengths while promoting their independence, choice and control.

We encourage positive risk taking with a ‘home is best’ philosophy, this is reflected in our performance as Telford and Wrekin is proud to have a lower than national average per population of admissions into long term placements for older people.

The post holder will work within the hospital team to support residents who are experiencing a rapid decline in their health and are at risk of being admitted to hospital.

The post holder will work with the individual, their carers and families, to build a Support Plan, with a broad range of interventions that will provide a provision that is holistic, evidenced based and delivered within communities.
