The Physical Disability and Sensory Service works with adults across Warwickshire. We are a new a service of committed staff working with adults with a physical disability and/or sensory.

We’re looking for a qualified Social Worker to join our service.  This is a busy team committed to meeting the needs of our customers and carers and providing a supportive and holistic assessment and care management process, using mobile working and technology to improve ways of working.

The Role

37.0 Hours per Week

Joining our new team, you’ll be managing a caseload and travelling the county.  Responsible for providing assessment and care management support to adults with a physical and/or sensory disability within Warwickshire you’ll be undertaking new assessments; transitions from Children's services; Safeguarding work and working with vulnerable adults.   You will work in a person centred way to promote personalisation and empower individuals to make choices, supporting them in achieving their current goals in aiding and enabling them. 

In addition you'll be communicating with a wide variety of clients, communities, partners and agencies as well as working on your own initiative, often in pressurised situations and help the wider team as necessary.

Key Requirements

To be successful in this role it’s important you have a professional Social Work qualification; are registered with the Health and Care Professions Council and have experience of working with customers with physical and or sensory needs.  It’s also important you have strong analytical and communication skills both written and verbal and can work on your own initiative as well as collaboratively with internal and external stakeholders. 

Strong planning and organising, with the ability to work to tight deadlines, often under pressure, whilst at all times maintaining composure and your professional stance, you’ll be used to thinking creatively and making difficult decisions.  With strong influencing, persuading and resilience you’ll be used to dealing with confidential information with tact and discretion; have good IT and recording skills.
