The Role:
An opportunity has arisen for a permanent post of Social Worker within the All Age Disability Service. This is a great opportunity for someone that wants to develop their skills across Adult Services and Children’s Services. As the Young Adults Team will be moving into Worcester Childrens First to join the new All Age Disability Service, this is a really exciting time to be part of the change. The AAD aims to make a difference to all children and young people with SEN and additional needs, and the new service will join together Children with Disabilities and the SEND service to provide a seamless delivery of support. The role will remain as a Transitions worker but with a multi-agency delivery model.
The Teamwork with young people with disabilities aged 16-25. The work is primarily with young people with Learning Disabilities, but also some young people with a physical disability or complex health needs, to transition from Children’s Services into their adult life. The work is very varied, and requires working in a multi-agency manner with Worcestershire Children First(WCF), Adult Services, Special Educational Needs Department (SEND), health colleagues, schools and colleges to ensure young people get the whole range of options they deserve when moving into adulthood. The work involves using Liquidlogic in both the adult and children’s systems and using both children and adults legislation effectively.
We cover all of Worcestershire, with some placements nationally so any applicant would need the ability to travel.
We would love to interview anyone that has a passion in this area combined with great social work skills.
