We have an exciting opportunity for a Team Leader to join the Hospital Social Care Management Team within Warwickshire. 

 

The Team Leader will be responsible for the Out of Area North team in the North of the County and will line manage Practitioners within the Out of Area Team. The post will be based in the North of the County.  The post holder will also cover Team Leaders within the Acute Hospitals and Discharge to Assess Team when required.

 

As Team Leader you will be expected to deliver an effective day to day service to ensure timely and safe discharges of customers from hospital with the appropriate care and support.

 

The Hospital Social Care Team is a well led service which has excellent relationships with health.  There will be opportunities for you to develop your skills and abilities and be involved in service developments to continuously improve quality and compliance with current and new legislation.
