We will continue to focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible. 

This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life and remain independent for as long as possible.
 

The role:

We are looking to recruit a part time, Service Assessors in our Triage team. The role is based in the office at Wildwood and is at present Monday to Friday. 

The main duties are: 
•    The effective planning of hospital discharges 
•    Allocating assessments to the most appropriate person based on their needs
•    Responding to urgent requests
•    Monitor assessment and therapy capacity
•    Input and monitor a number of data systems, e mails, spreadsheets. 
•    Link in with a variety of health professionals, hospitals, social work teams, brokerage, etc
•    Support enquiries from families, service users etc
•    Submit brokerage requests to arrange  POC 
•    Deal with complaints or any concerns from family or service users
•    Arrange urgent medication enquiries or reviews to the most appropriate person
•    To be the point of contact for advice and guidance form other staff members  

As the ideal candidate you must have a level 3 Diploma in Health and Social Care or considerable experience of working in a social care setting.  It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate, and empathetic.
