The role:

An exciting opportunity has arisen for an Advanced Social Work Practitioner to work within the Reablement Team which operates across the county. This is a full-time position, 37 hours per week. We are seeking a highly motivated individual who has a track record of strength-based approach and multi-disciplinary working.

The Reablement Team supports with the planned discharge of people home from hospital and then determines their reablement goals on a discharge to assess model to support and encourage independence long term. This may include involvement from OT’s, Physiotherapists, social care staff and other agencies. You will also be involved with supporting community reablement referrals to promote independence.

The social work team also support with the exit work from the service and arrange any on-going packages of care that are needed once people have completed their reablement goals with the service.

The team works with older adults and adults 18 years and over with a range of care and support needs.

You will manage a small team of staff and have a small number of cases that you will directly support.

The team works within the Care Act 2014 and the Mental Capacity Act 2005. A focus on Home First approach and utilisation of the 3 Conversation Model are key to our work. Along with relationship building with partner organisations and other teams within the council.
 

Main Duties and roles:

  • To operate as an Advanced Social Work Practitioner (ASWP) working across the Reablement team. Having line management responsibility for a group of staff, providing regular supervision. Supporting hospital discharges to the reablement service and onward care. Supporting front line staff with complex case work. To support frontline staff with developing skills and expertise, with a focus on strength-based assessments and positive risk taking. Advocacy and mental capacity skills are key. To participate in team meetings and lead reflective development sessions aiding in embedding best practice. To support embedding of procedures and guidance to daily practice and participate in development and maintenance of processes. There will be times where you may need to manage a small number of complex cases. To chair/ participate in meetings, these may involve complex discharge planning or best interest decisions. Developing and maintaining relationships across partners organisations, service providers and voluntary sector.

The successful candidate will be expected to demonstrate strong Leadership Skills and are committed to supporting the social care Workers development. It would therefore be desirable that you have completed PE1 and PE2 or working towards this. Or have experience in a leadership role where these skills are demonstrated. It is important that you are creative and innovative in your approach to ensure best outcomes are achieved.
