Teaching Assistant with Transition Admin Duties
(Fixed Term for One Year)
- Teaching Assistant – NJC Grade 5, £23,953 - £27,514 pro rata (actual salary £19,337 - £22,211) –
35 hours per week – term time only

We aim to help our young people overcome social, emotional and mental health needs (SEMH) in order to manage their behaviour and make considered choices.
We require a passionate and committed Teaching Assistant to work with young people and staff onsite at Lawnswood Campus.
Applicants will be expected to have:
- Experience of working with young people with SEMH and SEND
- A proven track record of good communication and organisation
- A willingness to undertake specialist training
- An interest in supporting students with medical needs within the hospital school room
Job Overview:
To support with the administration of transition in and out of all Centres on Lawnswood Campus.
Collaboration:
Working closely with Referrals and Commissioning Officer, Heads of Centre, Centre Admin Officers, Centre Attendance Officers and Behaviour and Attitudes Lead Teachers
Duties and responsibilities:
Referring in:
Set up a meeting with school for new School Referrals – discussion around your person and expectations for referral
Set up a meeting with schools for new PEXs – discussion around your person and background information
Set up an initial welcome meeting with young person and family and invite key staff (before 6th day for PEX)
Create Pupil Files online and on paper and communicate with THRIVE Induction lead
Liaise with SENDCos to address EHCP outcomes and needs analysis
Set up online reviews 5 and 10 weeks in Centre Review Calendar
Collect Pupil Files from Schools after 15 days PEX and PDC is completed.
Communicate dates for School Place reviews with schools at 5 and 10 weeks and invite school and Centre staff
Communicate dates for PEX Reviews with the LA Inclusion Team and invite Centre staff
Updating Risk Assessments after incidents
Referring out:
Making contact with the Exploitation HUB regarding police footprint for a potential referral to ISSAP
Circulating the Readiness for Reintegration Journey and Readiness for Reintegration Checklist for Staff
Making contact with the Inclusion tea for transition support
Checking the completed Readiness for Reintegration Journey and Readiness for Reintegration Checklist and sending to Head of Centre for final checks and submission to ISSAP
Setting up meeting with new school , young person and Centre staff to agree transition plan
Completing the transition plan agreement and sending to school
Communicating the transition plan to schools
Supporting visits to young people transitioning back to schools

Lawnswood Campus is home to three Pupil Referral Units, The Braybrook Centre, The Midpoint Centre and The Orchard Centre which includes The Nightingale Home and Hospital Service serving the schools in Wolverhampton as part of a city wide strategy to promote inclusion and positive behaviour in safe, secure, child-centred, positive and nurturing environments.
- Braybrook: Key Stage 3 children and young people with social, emotional and behavioural difficulties and includes a Student Support Centre and Outreach Service;
- Midpoint: Key Stage 4 children and young people with social, emotional and behavioural difficulties and includes a Student Support Centre and Outreach Service;
- Orchard: Key Stage 3 and 4 children and young people who require social, emotional or mental health (SEMH) support;
- Nightingale: KS2-KS4 specialist short stay Centre that works in partnership with key agencies to offer appropriate intervention to vulnerable students with medical and mental health needs in a supportive and structured setting;
- LITS: Lawnswood Integrated Therapy Service is specialist provision consisting of Counselling Psychologists and trained Counsellors offering a range of therapies which include Humanistic, Cognitive, Behavioural, Psychodynamic and Systemic modalities. The clinical work is underpinned by evidence- based practice.

- We communicate closely with parents, carers and school staff to help them to overcome the difficulties experienced by their child or young person and provide opportunities for them to work with their child or young person in reflecting on their behaviour, identify coping strategies and identifying how giving time and praise helps the behaviour change process.
