An opportunity to join an award winning team providing support to People with a Dementia Related in Dudley.

Hours: 37 per week, working 5 days over 7

Location: The main base for dementia advisors is The Crystal Gateway Dementia Centre in Amblecote, but as an Agile worker you will work mainly from home  or in the community.

The role of the Dementia Advisor is to improve information and support to people living with dementia and those people newly diagnosed.

The emphasis is on supporting the person to live well with their dementia either at home or in their own local community by providing an information and signposting service, including support to the immediate carers, families and friends of the people who are referred to the service.

Experience of working in a community care setting with an understanding of the needs of people with dementia and their carers is essential as well as knowledge of relevant legislation including the Care Act, the Mental Capacity Act, Deprivation of Liberty Safeguards and Health and Safety.

The role will include participating in a Duty Rota as well as undertaking social care assessments to arrange a personal budget for attendance at The Crystal Gateway Dementia service. The ability to produce documents and reports to evidence personalisation using IT systems is required.

You will work flexibly to meet the needs of people as well as have an ongoing commitment to support the continuing development of the Dementia Gateway service. The successful candidate will be well organised and self- motivated with excellent communication skills.

Qualified to NVQ Level 2 or an equivalent in Health and Social Care is essential along with an A dementia qualification or a willingness to undertake
