The day-to-day duties

To assess clients’ needs using the Bridges assessment procedure, including a narrative based assessment and network mapping exercise to understand client’s needs, visiting clients in their homes, hospital, hospice or other appropriate setting as preferred by client, enabling them to access support services to manage their domestic, social arrangements to maximise their quality of life at home.

To coordinate support as identified in the assessment, sourcing additional support where needed. The Care Co-Ordinator will ensure clients are able to access all appropriate services e.g. benefit advice, small grants etc and provide client with accurate information as appropriate.

Qualifications:

  • Good standard general education and qualification in basic skills. Qualifications in IT.

Experience:

  • Experience of working with members of the public in a health and social environment. Experience of working with individuals and groups in an informal environment. Experience of working with multi-disciplinary skills. Experience of working as an effective team player.

General Attributes:

  • Must be able to travel within Birmingham and surrounding areas, alongside our Tipton office. Must be able to have an understanding of dealing with people with a diverse background. Ability to work independently on an outreach basis within Birmingham including home and hospital visits.

This role will be subject to an Adult Workforce Enhanced DBS check.
