Job Details

We are looking for experienced, organised and adaptable individuals to join our new multi-agency Intervention and Transition Team operated in partnership by Worcester City Council, Worcester Citizens Advice Bureau, Caring for Communities and People (CCP), St Pauls Hostel and Maggs Day Centre, with the aim of ending the cycle of homelessness and rough sleeping in Worcester City. This innovative new approach will deliver joined up, person-centred, trauma informed support to individuals with multiple complex needs without the barriers of organisational or system boundaries.

As a Rough Sleeper Intervention & Transition Support Worker, you will develop a single, consistent, and trusted point of contact with the individuals being supported throughout every stage of their homelessness journey, taking a creative, open-minded approach and working positively and proactively with your colleagues as a team player, regardless of agency or organisation.

Your flexibility, empathetic but assertive approach and ability to build trusted relationships will be key to the success of this new model, which aims to not just provide support, but to change systems and deliver meaningful, sustainable outcomes for individuals.

You must have experience of working in partnership with a range of other statutory and non-statutory organisations to support customers with multiple complex needs. You will have experience of robust support planning, record keeping and information sharing and an excellent knowledge of legislation, policies and practises relevant to customers experiencing homelessness and rough sleeping, including housing, social care, mental health and substance abuse.

You will also have a working knowledge of a range of welfare benefits, as well as debt management and personal budgeting.

You must also have a commitment to safeguarding and adopting safe risk assessment and lone working practices, in accordance with the team’s policies and procedures.
