The Role

Worcestershire County Council is committed to supporting its residents to live healthy and independent lives in their community.
In order to achieve this, the Council, in partnership with NHS colleagues and our care providers, provide a Reablement service that will assist adults referred to the council for social care to regain their independence.
Our approach will be to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible.
This is part of a broader approach to ensure Worcestershire residents are healthier, live longer, have better quality of life and remain independent for as long as possible.
We have developed a new role to help us to deliver the service more effectively – Senior Reablement Worker.
As the ideal candidate you must have considerable experience of work in a health and/or social care setting, including assessment and working with both individuals and carers to improve their well-being. You will need to have access to a vehicle or transport that will enable you to travel within the County, possibly at short notice.
