Job Title: Short Breaks Reviewing Officer

The Role
Come and join this exciting journey integrating with our colleagues in SEND and YAT to ensure better outcomes for the Children, Young People and Families we work with. You will be part of developing new ways of working, forming new relationships and embedding pathways and processes.
You would be joining a small team who work alongside SEND, Education, Health and other commissioned providers to ensure effective Short Breaks for those families open to the service with timely assessments and reviews. You will contribute to the EHCP annual review process and will share a commitment to achieving positive outcomes for children and young people with additional needs.
We are looking for a dynamic and creative person to work with Children with a range of disabilities and complexities and their families to understand their family dynamics and support needs and use this knowledge to contribute to outcome focused SMART plans.
You will need to demonstrate good organisational skills, to ensure that you are seeing Children, Young People and their Families prior to their annual reviews, contributing effectively to multi agency meetings. You will have the skills to write child focused reports in a timely manner.
