We are looking for Social Workers in the newly created 0-25 Team, which provides Assessment and Support Planning for young people who have special educational needs and disability. This is an exciting opportunity for you to contribute to the development of a redesigned multi-disciplinary SEND service in Solihull.

Initially social work will be provided on the basis of age and you will primarily work with young adults. However, the team will develop and ultimately provide social work across age groups, providing the opportunity to develop your skills in working with children.

You will work closely with families and other professionals, including education and health colleagues across the service, as well as voluntary organisations, to find local and informal solutions to meet the needs of young people, to plan and prepare effectively for adulthood and assist young people to develop Education, health and care Plans and, where eligible, individualised care.

You will need:

• A professional Social Work qualification

• HCPC registration

• Demonstrable experience of supporting people to devise and implement plans

• Demonstrable experience of delivering person centred working including assessments and reviews

• Detailed knowledge of relevant legislation, including Care Act 2014, Mental Health Capacity Act 2005 (inc DoLS), MHA 1983 etc.

• Computer Literacy

You will also be expected to demonstrate an understanding of wider transition and disability issues and be able to transfer your skills to work with people with a wide variety of needs.

You will also need the ability to travel around the Borough to meet service needs.
