The Role

37.0 Hours per Week

Fixed term contract for 12 months.

We are seeking enthusiastic and committed Social Workers to join our Disability Service within Warwickshire. The team is currently based in Warwick and Bedworth and consists of Social Care Practitioners, Social Workers and Occupational Therapists.

All Social Work posts are expected to have experience in Learning Disabilities, Safeguarding and Multi-Disciplinary Team working inclusive of Assessment and Reassessment work completed under the Care Act 2014. You will be required to work with a variety of customers, including people with Learning Disabilities and Autism Spectrum Disorder (ASD) with physical disabilities, mental health; carers and their families and other professionals and young people with disabilities in transition from children’s to adult services. You will be required to undertake outcome focussed assessments, support planning, reviews, risk assessments and reports. Crisis intervention/Duty, Safeguarding, Community Deprivation of Liberty, Section 117, Transforming Care and NHS Continuing Health, work will be the main tasks undertaken within the role. Focussing on strengths and utilising an asset-based approach, you will enable customers to identify outcomes, meet their full potential and maximise their independence. As a strong team player, you will be required to contribute to the development and promotion of the Learning Disability Team, using imaginative, creative and innovative ways of meeting the support needs of young people and adults with Learning Disabilities. The roles will give the applicants the freedom to think creatively and design support plans tailored to individual’s support needs and outcomes.
