The new HomeFirst service was launched in January 2017. HomeFirst is an integrated health and social care service. The Service enables customers to remain as independent as possible for as long as possible with the focus on promoting independence. It aims to do this by supporting people to continue living at home, preventing admissions into hospital or residential settings, and facilitating timely discharges.
37.0 Hours per Week
You will be carrying out comprehensive assessments with customers who have either returned home from hospital following a period of ill health or who are struggling to manage their activities of daily living at home.
You will set up a plan with customers to enable them to relearn the skills needed to function more independently and review as required. You will identify any areas of need that are ongoing and refer onto appropriate professionals as required.
You will work as part of a team consisting of Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors and Reablement Assistants. You will be expected to provide professional support and guidance to the team and you will have professional support from a Team Leader.
We cover the service from 9 am - 8 pm.
The current shift patterns are 9 am - 5 pm and 12 pm - 8 pm Monday to Friday (we are working towards 7 day working for all staff).
- Excellent communication skills and time management skills are needed due to the fast pace nature of the service.
- Experience of completing comprehensive assessments including complex moving and handling assessments is required.
- Knowledge and experience of using a range of equipment to support functional ability is needed.
- Computer skills are important as we use a range of databases/computer programmes to communicate and document our interventions.
