This project has been commissioned by South Warwickshire Place which focuses on improving the lives of people within the areas of Warwick District and Stratford-upon-Avon District through partnerships and collaboration from organisations supporting health and care needs (such as local government, health, housing, social care, Healthwatch and VCSEs).

We are mobilising an innovative project between the Council and health partners in Warwick District to improve respiratory health in Lillington. This project aims to work with the community and wider stakeholders such as GPs, social prescribers and specialist respiratory nurses to improve awareness and management of respiratory conditions such as asthma, COPD and hypertension which are more prevalent in priority neighbourhoods, such as Lillington and have a significant impact on quality of life.

We are looking for a motivated individual who has some experience of working with health partners. The role of Community Connector will join our Community Wellbeing Team in Housing and aims to undertake work specifically in supporting individuals to engage in activity that improves their respiratory health and wellbeing. The post is fixed term and will be instrumental in leading and coordinating this work from the new Lillington Health Hub, opening May 2024. As well as focus on promoting healthy lifestyles and smoking cessation a key area of focus will be on the link between housing and health.

The intention is for the postholder to work flexibly between home and the new Lillington Health Hub as well as out in the community and attending external meetings and events.
