We are looking for an experienced social care manager to lead our newly formed Community Promoting Independence Service for people aged 18+, with care and support needs. The is an exciting opportunity to work in a multi-disciplinary team, focusing on promoting wellbeing and choice, making a real difference and enabling people to improve their independent living skills.
This Team Leader will be required to lead the newly formed Community Promoting Independence Team, working with people aged 18+ with care and support needs. This will involve line managing a small team of Occupational Therapists, Social Workers, and a Broker, and working closely with key stakeholders including health colleagues, care agencies, and third sector colleagues. The postholder will be required to comply with relevant legislation, including the Care Act, the Mental Capacity Act, and the Mental Health Act. Postholders will be expected to promote asset-based social work, and support people to contribute to their community.
Fixed Term until 31st August 2018.
• A qualification in occupational therapy or social work
• Current registration with HCPC
• Staff management and development skills and experience
• Financial and budget management skills
• Sound report writing skills
• Experience of care management for people aged 18+
• Experience of promoting independence
• A high level of organisational skills
• Sound communication skills, including written skills
• Excellent interpersonal, negotiation, problem solving and analytical skills
