Job Details
As an exemplary practitioner, you will be required to provide support, advice and share professional knowledge to colleagues; promote innovation and model and facilitate reflective practice.
Hours: 2 permanent posts, 37 hours. Part time considered.
Salary: Grade 8 , £33,024 to £35,745 per annum
Location: Various locations across the Dudley borough and some home working.
The post will involve working with adults and this may involve vulnerable adults. You will be required to undertake assessments and develop support plans to enable them to achieve an optimum level of independence to develop and maintain relationships and be valued members of their community.
Due to the requirement of the service, you will be asked to work flexible hours and participate in a 7-day rota covering some weekends and a set number of bank holidays for which relevant enhancements will be paid.
Demonstrate professional commitment and a high level of knowledge, critical thinking, communication skills and creativity, in order to support people to achieve positive outcomes and a good quality of life.
As a social worker, you will be involved in working with multi-agencies to facilitate safe discharges following different pathways as necessary.
You will also be asked to undertake duty and respond to front of house to facilitate immediate discharges for those who are not liable to be admitted.
This role will involve assessing service users and attending both residential and nursing homes.
