About Us

 

Warwickshire County Council is pleased to announce due to expansion of the service we are looking for Occupational Therapist’s to join Warwickshire’s Reablement team.  We are a well-established team of Occupational Therapists, Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors, Reablement Assistants and Social Care Practitioners, who have provided years of interventions for Warwickshire Reablement customers and aim to achieve the best outcomes going through their Reablement journey.

 

Reablement focuses on promoting independence by supporting customers to do as much as possible for themselves both physically and psychologically. We do this by supporting customers to continue living in their own home, preventing admissions into hospital or residential settings, and facilitating timely discharges home.

 

We believe that by supporting customers to identify and achieve their personal goals and outcomes, and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on health and well-being.

 

The Role:

 

Reablement Occupational Therapist’s complete comprehensive assessments with customers who are struggling to manage activities of daily living in their homes or who have returned home from hospital following a period of ill health. Reablement customers have the opportunity to work on outcomes that are important to them.

 

You will be working within the service to respond preventatively to referrals for Adult Social Care and support and will use Occupational Therapy interventions and a strengths-based approach to enable adults and older adults to remain in their own homes with the optimum level of independence, and to enable / support customers to achieve their desired outcomes.

 

You will provide professional support and guidance to the team, using knowledge and expertise for complex cases. You will receive professional support from a Senior Occupational Therapist/Occupational Therapy Team Manager. You will work within the requirements of the Care Act 2014, HCPC codes of practice and other relevant legislation and guidance.

 

Key Requirements:

 

To be successful in this role you will have experience of working with a range of people with different needs promoting independence. It is desirable for the Occupational Therapist to have previous knowledge and experience in moving and handling risk assessments and developing Reablement outcome plans. 

 

Excellent communication skills and the ability to work within a team as well as on your own initiative is vital to this fast-paced role.  Having good time management/organisation skills is required; being solutions focussed; flexible in your approach, willing to travel across the county as needed, as well as having experience of handling confidential information with tact and discretion. 

 

The post requires you to work a shift pattern of 9 am - 5 pm and 12 pm - 8 pm Mondays to Fridays on a rota basis, including one Saturday in every 7 weeks.  There will also be requirement to work bank holidays on a rota basis.
