Social Care Worker
SEND Social Care consists of an Operations Manager, Team Managers, Social Workers, Social Care Workers and administrative workers. You would be joining a very strong leadership team who are working together to deliver the Authority’s transformation agenda. We are supporting our staff to deliver services which are able to enhance the life experiences and chances of children and young people with a disability, and their families, and keeping them safe and healthy. As a team we are recognised, both within our own agency and outside, a genuine commitment to getting the best outcomes and seeking excellence as a standard of work.

37.0 Hours per Week
You will be required to work with a variety of children, young people and their carers, including people with learning disabilities and Autism Spectrum Disorder (ASD), physical disabilities and mental health. You will support young people with disabilities in their transition from children’s to adult services. You will be required to undertake outcome focussed tasks, support planning, reviews, risk assessments and reports, crisis intervention/ duty, safeguarding. Focusing on strengths and utilising an assetbased approach, you will enable children to identify outcomes, meet their full potential and maximise their independence.
