Senior UPI Assistant
37 Hours - Permanent 8 week rota comprising weekends, evenings and day shifts
The Urgent PI Service provides support to individuals who live in their own home. The service is registered with the Care Quality Commission and is based at Wildwood, County Hall Campus. This service is provided across the whole of the county of Worcestershire and teams operate within specific areas so the need to travel is essential. We require a permanent Senior Homecare Assistant to support Service Users and carers in their own homes, liaise with other professionals and maintain good working relationships within the Health and Social Care Directorate. This also includes first visits, transcribing and updating medication sheets and undertaking quality monitoring visits. The aim of the service is to provide support to individuals so they can remain in their homes and to promote their independence following hospital discharge or crisis intervention and, to observe during a period of assessment any ongoing service requirements that would aid recovery. You should have a commitment to developing the lives of people, maximising their levels of independence and providing care and support that meets individual outcomes. You need to be an effective communicator and experienced in multi-disciplinary working You should have the ability to travel independently, work autonomously and be a good team player. You need to have excellent organisational skills and experience of managing and leading teams. You will need an understanding of maintaining the health & safety and wellbeing of service users in their own home. The service is provided from 07.00 to 22.00. It is centrally managed through a Call Centre based in Worcester, which is open from 06.30 to 22.00. The post holder would take part in the 8 week rolling rota which covers 7 day working.
The rota will include some weekends and Bank Holiday working (Bank Holidays paid at Double Time) to provide cover 365 days of the year.
