The Role
Would you like to be a part of the Social Care Service? Would you like to support hospital patients to return home as quickly & safely as possible? Would you like to help to maintain the health,  safety and wellbeing of people in their own home?  Do  you have good communication skills and excellent organisational skills?  Are you able to  report and record information accurately?  Are you prepared for a challenging role? If you can answer 'yes', then please read on.

Worcestershire County Council is committed to supporting its residents to live healthy and independent lives in their community. In order to achieve this, the Council, in partnership with NHS colleagues and our care providers, provide a Reablement service that will assist adults referred to the council for social care to regain their independence.

Our approach will be to offer short-term intensive support to enable individuals to learn how to live their lives as independently as they can and support them to regain independence during the period of support. We focus on what people can do and build on these skills. We will make sure people receive the right support at the right time to help them live independently at home for as long as possible.

We are looking to recruit a permanent Co-ordinator who will be responsible for planning the programs of work for Reablement Assistants using our IT systems. As the ideal candidate you must have experience of multi-tasking, working to tight deadlines and being able to think on your feet. The reablement service is fast-paced and constantly changing so therefore you need to be able to react quickly and efficiently. It is also equally important that you can demonstrate a can-do attitude, and that you are proactive, compassionate, and empathetic.

The Reablement Contact Centre is open from 07:00 until 22:00 365 days of the year.  You will be working a 7.5 hour day ( excluding lunch break) on a 8 week rota that includes 2 weekends, evenings & public holidays.

In return you will be supported in your role by your line manager through one-to-one meetings and performance reviews, team meetings and will have access to the corporate wellbeing offer. You will also receive 5 weeks annual leave in addition to Bank Holidays, rising to 6 weeks after 5 years’ service, and access to the Local Government Pension Scheme.

If you are interested in this role, please submit an application detailing how you meet the requirements of the role. You should refer to the attached Job Description and Person Specification to support this.

If you require further information, please contact the Reablement Contact Centre on 01905 822876 and ask for Sue Beckett or Karen Baldwin.
