Location: County Hall, Worcester

The Role
The Adult Front Door Service is the first point of contact for individuals seeking health and wellbeing support.
The Adult Front Door Social Worker will be responding to incoming portal referrals via phone and offering advice to team members in respect of the Care Act 2014.

Our aim is to prevent or delay individuals from developing longer term care and support needs by enabling them to be healthy for as long as possible, self-managing long-term conditions.
