Grade: G4: £23,893 - £25,979 per annum (pay award pending)

Community Reablement Co-ordinators: This is a pivotal role within the service as you will be the conduit between the practitioners, care provider and residents. Great IT and communication skills are a must.

Are you a confident and effective communicator who enjoys working with others? Do you have good IT skills, being able to use different software programmes? Do you thrive in a busy environment and have the skills to work on your own initiative as well as being a team player? If so, then this is an exciting opportunity to join our new service.

About the role

As a Community Reablement Co-ordinator you will play a vital role through brokering services, communicating with external providers, colleagues and residents to deliver and maintain a high standard in service provision. Utilising your IT skills will ensure information is accurately recorded on the appropriate systems relating to purchasing of services.
