About Us: At Walsall Metropolitan Council we are committed to providing high-quality care and support to vulnerable adults across all age spans. We adopt a strengths-based approach, focusing on empowering individuals to live as independently as possible.
Role Overview: As a Social Care Facilitator, you will play a crucial role in supporting and assessing vulnerable adults. Ensuring that each individual receives personalised support tailored to their unique needs and strengths. You will work closely with other professionals and be required to undertake statutory duties in line with the Mental Capacity Act and the Care Act.
We currently have posts available in the following teams:
- Learning Disability Team
- Younger adults Team (18 - 25)
- Localities
- Mental Health Team

Key Responsibilities:
- Conduct strengths-based assessments to identify the needs of individuals and their carers.
- Develop and implement outcome focused personalised support plans.
- Review support plans.
- Provide ongoing support and monitoring to ensure the well-being of individuals.
- Collaborate with multidisciplinary teams to deliver comprehensive support.
- Advocate for the rights and needs of vulnerable adults.
- Maintain accurate and up-to-date records.
Qualifications:
- Relevant qualifications in social care or a related field and/or an understanding of Adult Social Care legislation.
- Experience in working with vulnerable adults.
- An understanding of safeguarding adults.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Commitment to promoting independence and well-being.
