Job Title: Adults Social Worker
Team: Discharge to Assess (D2A) / Urgent Care
Salary: £31,067 to £40,476 per annum + £3000 Market Forces*
37 Hours per Week

The Role
The Community Hospitals Team have an exciting opportunity for an Experienced Social Worker, and we are keen to hear from you. On this occasion we will not be accepting applications from Newly Qualified Social Workers. We are a large supportive team supporting patients in the Community Hospitals and those in Discharge to Assess Beds in Nursing Homes. We work with our Colleagues in the Acute Hospitals both in and out of county.
This post sits within the Discharge To Assess part of the service so you will be working with some of the patients with complex care needs and as such a working knowledge of CHC is key as is Care Act eligibility. If you are successful, you will be working with a number of people who are cognitively challenged so you will need sound knowledge and experience of completing Mental Capacity Assessments.
Safeguarding and working positively with risk are part of role as well as understanding the need to support Carers and Families at what can be a stressful overwhelming time.
While we are keen to minimise unnecessary road journeys the Nursing Homes are Countywide so you will need to be able to travel throughout the County. We also have 21 D2A Beds based in the Community Hospitals so if you enjoy working in a Multi-Disciplinary manor this could be the job for you.
We have several Practice Educators within the Team so Social Workers have plenty of opportunity to share their knowledge and learning and we pride ourselves on the support and camaraderie the Team provides. The Team meets regularly in Team Meetings where we have guest speakers and in smaller cluster meetings with ASWP’s where workers have an opportunity to share knowledge and learning and aspects of good practice. You will have regular Supervision and peer support to ensure you are able to continue your career development and career aspirations.
