HomeFirst/Reablement Team 
This team focuses on promoting independence by supporting people to do as much as possible for themselves both physically and psychologically. It aims to do this by supporting people to continue living at home, preventing admissions into hospital or residential settings, and facilitating timely discharges.

We believe that by supporting customers to identify and achieve outcomes in every aspect of their lives and by embedding personalisation, customers will have choice and control over how their support will be delivered, this will have a positive impact on health and well being.

Personalisation Service 
The Occupational Therapy Personalisation Service is an exciting and collaborative service. We’re at the forefront of the preventative and early intervention agenda for Warwickshire County Council, District and Borough Council and discharge pathway with health. Our purpose is to enable customers and their carers to live as independently as possible by providing practical solutions that reduce or remove the need for ongoing care, through focused multi agency interventions. 

The Personalisation Team work with adults who have long term physical and/or cognitive disability and we’re looking for an Occupational Therapy Assistant and Occupational Therapist to join us on a 12 month, fixed term basis.

Learning Disabilities 
The Occupational Therapy Learning Disabilities Service is an exciting and collaborative service. Working with our Social Work colleagues we provide a specialised skill assessment service to enable our customer to achieve their maximum potential and independence. This can be in their own or family homes which we can assess and refer onto the District and Borough Council housing service for adaptations or in the purpose built extra care housing facilities being developed by the County across Warwickshire. Our purpose is to enable customers and their carers to live as independently as possible by providing practical solutions that reduce or remove the need for ongoing care, through focused multi agency interventions.

The Learning Disability Team OTs work with adults of all ages who have a learning disability and are now looking for an Occupational Therapist to join us on a 12 month, fixed term basis.

The Role

37.0 Hours per Week

We have for 4 roles across HomeFirst/Reablement, Personalisation Service and the Learning Disabilities Team:

HomeFirst Reablement:
1 x Occupational Therapist (Permanent, part time - 32 hours). We currently work shift patterns 9am-5pm and 12pm - 8pm and we are working towards providing a 7 day service. Base will be at Heathcote House in Leamington however as we are a countrywide team you will be expected to support the North HomeFirst Reablement team if required.

You will be carrying out comprehensive assessments with customers who have either returned home from hospital following a period of ill health or who are struggling to manage their activities of daily living at home. You will set up a plan with customers to enable them to relearn the skills needed to function more independently and review as required. You will identify any areas of need that are ongoing and refer onto appropriate professionals as required. You will work as part of a team consisting of Occupational Therapy Assistants, Reablement Officers, Reablement Supervisors and Reablement Assistants. You will be expected to provide professional support and guidance to the team and you will have professional support from a Team Leader.

Personalisation Service: 
1 x Occupational Therapy Assistant (12 month fixed term, full time 37 hours) 
1 x Occupational Therapist (12 month fixed term, full time 37 hours).
Base will be at Saltisford or Kings House with an expectation to be able to work countywide. Core working hours are Monday to Friday 9am - 5.30pm, however, all applicants are expected to be willing to undertake weekend working should the core service hours change.

As an Occupational Therapist or OT assistant for the Personalisation service, you’ll be working in a multiagency environment and undertake assessment of needs as directed by the Senior Occupational Therapist. Travelling across the county, you’ll be managing a caseload of individuals with long term physical and/or cognitive disabilities; helping to carry out functional, risk and standardised assessments; developing support plans with customers and carers; supporting the implementation of Occupational Therapy Clinics; and helping the wider team as necessary. You’ll be responsible for prescribing assistive technology, equipment and minor adaptations to provide effective OT interventions to support people to remain within their own homes. As an Occupational Therapist, your caseload will involve working with people with more complex long term support needs.

Learning Disabilities Team:
1 x Occupational Therapist (12 month fixed term, full time 37 hours). Base will be either Saltisford, or Kings House with an expectation to work countywide. core working hours are Monday to Friday 9am - 5.30pm, however all applicants are expected to be willing to undertake necessary visits outside of these core service hours. Warwickshire County Council also operate a Family Friendly Flexible Working Policy.

As an Occupational Therapist in the Learning Disability service, you will be working in a multiagency environment and undertake assessment of needs as directed by the Senior Occupational Therapist. Travelling across the county, you will be managing a caseload of individuals with complex needs and disabilities; helping to carry out functional, risk and standardised assessments; developing support plans with customers and carers; and helping the wider team as necessary. You will also be responsible for undertaking basic skill assessments, prescribing assistive technology, equipment and minor adaptations to provide effective OT interventions to support people to remain within their own homes and or care facility.

Key Requirements

HomeFirst/Reablement:
Excellent communication skills and time management skills are needed due to the fast pace nature of the service. Experience of completing comprehensive assessments including complex moving and handling assessments is required. Knowledge and experience of using a range of equipment to support functional ability is needed. Computer skills are important as we use a range of databases/computer programmes to communicate and document our interventions.

Personalisation Service:
To be successful in this role you’ll have an experience of working with a range of people with different needs promoting independence. As you’ll be managing a variety of relationships it’s important you have good communication skills and have the ability to work within a team as well as on your own initiative. This is a busy role so you’ll also need to be well organised; solutions focussed; flexible in your approach, willing to travel across the county as needed, as well as having experience of handling confidential information with tact and discretion.

It’s desirable for the Occupational Therapist role to have previous knowledge and experience in moving and handling risk assessment, and developing moving and handling support plans.

Learning Disabilities team:
To be successful in this role you’ll have an experience of working with a range of people with different needs promoting independence. As you will be managing a variety of relationships it’s important you have good communication skills and have the ability to work within a team as well as on your own initiative. This is a busy role so you’ll also need to be well organised; solutions focussed; flexible in your approach, willing to travel across the county as needed, as well as having experience of handling the challenges of the role and a variety of confidential information with tact and discretion.

It would be desirable for the successful candidate for this Learning Disabilities Occupational Therapy post to have previous knowledge and experience of working with customers with a Learning Disability.
