Welcome to Telford and Wrekin – Simon Froud – Director of Adult Social Care

The role
We are seeking a Service Delivery Manager to lead and support the Adult Social Care responsibilities for admission avoidance and discharge from hospital. This pivotal role involves overseeing the delivery of high-quality Intermediate Care or Enablement services ensuring seamless coordination between hospital and community care and support.
Key Responsibilities
- Lead and manage teams that support avoidable admission, discharge from hospital and case management to deliver exceptional adult social care services.
- Ensure effective communication and collaboration between hospital staff, social workers, and community care providers.
- Implement and monitor care plans to meet the diverse needs of patients transitioning from hospital to home or other care settings.
- Maintain compliance with relevant legislation, policies, and procedures.
- Oversee the allocation of resources, including staff and budget management.
- Maintain accurate records and prepare detailed reports on service delivery and outcomes.
- Develop and maintain strong partnerships with the Integrated Care Board to enhance service integration and patient outcomes as a key point of contact for ASC.
- Take on Lead roles across ASC to ensure there is a strategic focus with operational delivery.
You will need
- A professional qualification such as Social Work, Registered Nurse or Therapist
- Proven experience in a leadership role within adult social care or healthcare settings
- Strong understanding of hospital discharge processes and community care services
- Excellent communication, organisational, and problem-solving skills
- Ability to work effectively within a multidisciplinary team
- Experience in partnership working with health and social care organisations
