Job Details

Are you an experienced Social Worker, registered with Social Work England and have worked in a busy Hospital/Intermediate Care Team? Then come over to Sandwell as we have lots of opportunities.

We are looking for enthusiastic and committed professionals to join our team, to help shape and deliver our service, and contribute to the development of a model of best practice. To do so, you will be supported by a strong leadership team.

This is an exciting time to join Sandwell’s Council’s Adult Social Care Service. As an Adult Practitioner, you will have the opportunity to oversee day-to-day operations within the Hospital and Intermediate Care Team.

We provide a high- quality service in relation to all aspects of the assessment and care management process, and you will:

  • assist the Social Care Team Manager with providing professional supervision to other team members.
  • undertake prioritisation of enquiries from our customers, and ensure a timely and proportionate response to requests for Care Act Assessments.
  • be required to demonstrate an excellent knowledge of the Care Act 2014 and related legislative frameworks. 
  • supervise staff on a day- to- day basis, work in partnership with NHS and other professionals, ensuring that you have a strong customer focus.
  • need to be motivated and flexible with high professional standards and sound knowledge and experience of current legislation including the Care Act 2014, Continuing Health Care, Mental Health Act, Mental Capacity Act 2005, and Community DOLS/LPS.
  • possess a Social Work Qualification (DipSw, Social Work Degree or equivalent), along with Social Work England Registration.

The successful applicant will undertake an enhanced DBS check.

We are particularly looking for an enthusiastic and committed professional to join our team to help shape and deliver our services and contribute to the development of a model of best practice. In order to do so, you will be supported by a strong leadership Team and a committed Social Workers, Social Care Officers.

We have vacancies within the hospital/intermediate care team which is an exciting opportunity for individuals who are flexible, innovative and have a proven track record of partnership working, with a can-do approach.

We operate a 7-day service including Bank Holidays. You will be one of 7 managers who will support the Team Manager within the 7-day Hospital intermediate care team. You will work from various locations and will ensure the timely discharge of people from hospital or intermediate care settings.

Hours 

37 hours per week.
