We are seeking an experienced Social Worker to join our expanding Social Care and Support Service. This role involves collaborating with health professionals to facilitate the discharge of customers from hospitals under the Learning Disability and Autism (LDA) initiative, formerly known as the Transforming Care Agenda.

 

Key Responsibilities:

 

  • Collaborate with health professionals to facilitate hospital discharges.
  • Support adults with learning disabilities and autism to achieve their desired outcomes.
  • Utilise strength-based approaches to maximise positive outcomes for individuals.
  •  

Requirements:

 

  • Proven experience as a Social Worker.
  • Experience of working with children or adults with a learning disability and/or autism
  • Strong commitment to improving outcomes for adults.
  • Enthusiastic, dedicated, and passionate about social care.
