The Role:

The Reablement Service assists people requiring additional support and rehabilitation on discharge from hospital, and those in the community who need increased care or therapy input. Assessments are holistic and focused on maximising independence at every stage.
As a member of the Reablement team, you will work closely with other therapists and social care teams. You will complete holistic, goal focused, person centred assessments, and develop thorough treatment programmes to meet the goals. You will manage a varied caseload, spanning all Occupational Therapy specialities.
You will work closely with both health and social care colleagues, in a blended approach to care. Physiotherapists, Nurses, Occupational Therapists, Therapy Assistants and social care staff undertake collaborative working daily to ensure that all of a person’s needs are met. Regular clinical supervision and professional support are provided. We positively encourage any special interests which complement the patient demographic that we see.
The post holder will:
• Undertake Occupational Therapy assessments and provide therapeutic intervention for a designated caseload, working closely with individuals to identify goals as part of their overall care plan.
• Provide case management support for a designated caseload, with guidance from the Senior Occupational Therapist.
• Develop and co-ordinate individualised rehabilitation plans in close liaison with the team, individuals, their carers as well as all other services.
• Enable individuals to achieve maximum independence with activities of daily living, providing appropriate direction and guidance to Therapy/Promoting Independence Assistants in the service to support an individual’s rehabilitation and reablement where able.
In return you will be supported in your role by your line manager through one to one meetings and performance reviews, team meetings and other corporate wellbeing tools including an Employee Assistance Programme. You will also be provided with regular clinical supervision. You will also receive 5 weeks annual leave in addition to Bank Holidays, rising to 6 weeks after 5 years’ service, and access to the Local Government Pension Scheme.
