Job Title: Social Worker
Salary: £32,115 - £40,476 per annum, plus £3000 market forces
Hours per Week 37
Location: Wildwood , Worcester
Team: Permanent
The Role: We are offering an exciting opportunity for a Social Worker to join out Adult Pathway Planning Team!
Adult Social Care Commissioners are currently working on a Social Care Mental Health Recovery Pathway and a Supported Living Project. Commissioners need to work alongside co-ordinated Social Work Reviews to clearly define current service provision and support moves to more independent living where appropriate and we have established a Pathway Planning Team to support the work.
Key priorities for the successful candidate in partnership with our commissioners will be:
Providing Social Work input and skills to the pathway planning team with knowledge and experience of needs assessment, reviewing support plans and an understanding of The Care Act, Continuing Health Care, and s.117 Aftercare provision (MHA)
Support the development of recovery pathways for Mental Health enabling more individuals to live as independently as possible.
Comprehensively review packages for individuals who may be able to move/change services.
Identify if individuals can step down or move to alternative community living and support such changes.
Support re-modelling/de-registration/de-commissioning of services.
