The Role
We are looking for an experienced social worker for the position of Advanced Social Work Practitioner: Mental Health Discharge Co-ordinator, which is a new role created after a highly successful pilot project for the adult acute mental health wards. This post is primarily working with working-age adults with functional mental health conditions, to plan safe discharges from the acute psychiatric wards at the Elgar Unit at Worcester Royal Hospital. Although, some cover will be required for the functional older adult acute ward at times to cover annual leave, and occasional travel to Mortimer Ward in Hereford.
As this is a new post there is the exciting opportunity to be a key part of the development of the discharge pathways from the mental health wards, to ensure we are offering our service users the best possible support. You will also have the opportunity to work with individuals at a critical point in their care planning and make a real impact on their quality of life.
The feedback from the pilot project so far has been incredibly positive, and we are thrilled to be able to expand the team by a third practitioner.
Within the role you will be expected to assess needs under the Care Act 2014 and S117 Aftercare under the Mental Health Act 1983, complete any relevant capacity assessments, and work closely with our health and partner colleagues.
You will need to be able to work autonomously and proactively as you will pick-up referrals directly from the ward and engage with individuals at the earliest opportunity to support discharge planning.
Ideally the candidate would have some experience in mental health, but support and training can be provided by the mental health teams to support your development.
You will be part of a small Mental Health Hospital Discharge team (joining 2 other ASWPs doing the same role) and will work closely with colleagues in Mental Health North and South teams. You will be welcome to attend reflective practice and team meetings for both teams as while this role is inpatient based you will work with individuals from across the county. You will also work closely with our colleagues from Herefordshire and Worcestershire Health and Care Trust are also excited to see the development of this post, and to work alongside us to ensure that you are a welcomed and valued part of the team.
