Level 3 Social Workers  -  Adults Urgent Care Rapid Response

1 x based at Evesham Community Hospital covering the Evesham and Pershore area

1 x based at Kidderminster Health Centre covering the Kidderminster and Wyre Forest area

37 hours per week - 2 late shifts per month (12 Noon – 8pm)

1 in 6 weekends  - 12 noon – 8pm Friday,    9am – 4 pm Saturday and Sunday

Salary up to £31,601 plus 10% for out of hours rota

Working in a dynamic multi-professional environment you will be working collaboratively with NHS and Adult Social Care colleagues to ensure that people receive the care and support necessary to meet their needs in the most appropriate setting – right care, right place, right time, and ensuring that effective patient flow is maintained across the health and social care system

You must be committed to person centred practice and have detailed knowledge of relevant legislation including the Care Act and the personalisation agenda. You must also have a detailed understanding of continuing healthcare.
