An opportunity to join an award winning team providing support to People with a Dementia Related need in one of two Dementia gateways in Dudley.

Hours: 37 per week, working 5 days over 7

Location: Based within Brett Young Centre, Halesowen, but covering the community across the borough

The role of the Dementia Advisor is to improve information and advice to people either newly diagnosed with dementia or people formerly diagnosed with dementia and to be a named contact for the person and their family and carers throughout the duration of the condition.

The emphasis is on supporting the person to live well with their dementia with a requirement to provide an information and signposting service to immediate carers, families and friends of the people who are referred to the service.

Experience of working in a community care setting with an understanding of the needs of people with dementia and their carers is needed as well as knowledge of relevant legislation including the Mental Capacity Act; Deprivation of Liberty and Health and Safety.

The role will include undertaking social care assessments to access internal services or to arrange a personal budget for attendance at one of the Dementia Gateway centres, which requires an ability to produce documents and reports to evidence personalisation using IT systems.

The ability to work flexibly to meet the needs of people as well as an ongoing commitment to support the continuing development of the Dementia Gateway service is required.

Qualified to NVQ Level 2 or an equivalent in Health and Social Care is essential along with an accredited qualification in Dementia.

Well organised, self motivated with good communication skills.
