Care Navigators (High Intensity Users – North Warwickshire)
Age UK Warwickshire is a leading charity in the county providing a wide range of services to older people.
Working with GP Practices and Interdisciplinary Hubs across North Warwickshire to identify unmet needs, providing information and advice about a range of services to promote health and wellbeing and maintain independence to achieve holistic support planning and co-production of person-centred outcomes.
Candidates will have at least 2 years’ experience of working in health, social care or information and advice and in direct contact with people, families and carers in paid or voluntary work. Car driver essential.
The post holder will support service users identified as High Intensity Users. Working in partnership with service users the post holder will seek to appropriately reduce the number of unnecessary A&E calls, emergency ambulance calls and unscheduled care interactions. Improvements will be used to deliver measurable outcomes for the benefit of patients, staff and the public.
The post holder will lead in removing potential barriers and stigma associated with High Intensity Users to promote equality, diversity and safeguarding service-wide. Acting as an advocate the post holder will support service users reducing the need to access unscheduled hospital visits.
