This is an exciting time to join Walsall Council’s Adult Social Care service.  We have recently appointed a new and forward thinking leadership team that has brought a renewed focus on professional practice development across the workforce, to ensure we promote positive and improved outcomes for all our service users.

Social Care Facilitators – Ref: SS41518
£25,212 - £29,614 per annum

We are currently looking for enthusiastic and motivated individuals with experience of working in social care to join our multi-disciplinary Locality Social Work and Learning Disabilities and Transitions Teams.  Job share can be considered.

North Locality Team

1 post working 37 hours per week

The North Locality Social Work team is a multi-disciplinary team working with Occupational Therapists, Occupational therapy assistants, Social Care Facilitators and Social Worker’s. We are working towards more integrated working with Health services including District Nurses and Mental Health. Developing your skills and working towards your professional goals is important to us, and we strive as a team to provide the best outcomes for the people we support and their carer’s.

As a social care facilitator, with a good knowledge of the Care Act 2014 and associated legislation you will have the important role of completing initial assessments, reviews and promoting the preventative agenda of the Care Act 2014. A Social Care Facilitator with a background in working as an occupational therapy assistant would be an advantage.

Learning Disabilities and Transitions Team

1 post working 37 hours per week
1 post working 18.5 hours per week

You will need to have a good knowledge of the Care Act principles and be able to work autonomously in the community to support service users and their carers completing home visits, assessments/reviews, and support planning and carer’s assessments.  You need to be aware of the signs of abuse and safeguarding principles and responsibilities under the Care Act.  You need to have good practice knowledge of the Mental Capacity Act, be able to complete Mental Capacity Assessments and Best Interest decisions and recognise DOLs criteria.

The social care facilitator posts within the Learning Disabilities and Transitions Team will require some knowledge and/or experience of supporting people with learning disabilities and their carers. You will also need good assessment and support planning skills and the ability to work as part of a team.

In return, we will offer access to supportive experienced Team Managers and Advanced Practitioners, regular supervision, good peer support and a buddy system to support you.  We offer staff development through a range of training/support methods.
